Please fill out the form below and print it. You can then send it along with your media. You may pay with check, money order or with PayPal (remember you don't need a PayPal account to pay with your major credit card or bank account). If you plan to pay with a major credit card, do not include the information with your order. Once we've completed your work, we will send you an invoice via email and you can simply and safely pay with your major credit card or bank account.

Please remember to attach a text description of how you want you discs labeled. If you want a picture imprinted on the disc (covers the entire disc), please include in your order or you may email us a file. Custom designed disc labeling is available for $40/hour.

For prints and 35mm slides, you may write the number with a circle around it on the back of the photo or write the number on a post-it note. By mailing your media and placing an order, you agree to the following conditions. Digital Media Solutions, Inc. will make every effort to treat your order with the highest level of importance and will try to ensure your media safety in every way possible. However, Digital Media Solutions, Inc. will not be liable for any loss, or damage of videos. In the event that a video is lost or damaged by us, we will only be liable for the cost of the blank media.

By mailing your media, you assume full responsibility for obtaining releases when required for sale, duplication and distribution. You hereby agree not to send any DVD, CD, analog tape or any other media that is copyrighted. You further hereby agree that the handling, processing and packaging is without warranty or liability, even though defects, damage or loss by negligence or other fault may occur. Furthermore, you agree to hold harmless and indemnify Digital Media Solutions, Inc. for any and all liability and/or claim arising from this transaction. If paying by credit card, you authorize the charges for the costs associated with this order including shipping/handling and any applicable taxes. You also understand that video transfers, audio transfers, DVD photo albums and duplications are not refundable due to incompatibility.
